Summary As a Social Worker at Age-Wise Solutions Inc., you will play a vital role in supporting the emotional and psychological well-being of residents and their families. This position offers the opportunity to work with a diverse caseload, addressing issues such as loss and grief, mental health, and family dynamics. Responsibilities - Provide psychosocial support to residents in long-term care and retirement homes. - Conduct assessments to identify the needs of residents and develop appropriate intervention plans. - Collaborate with families and caregivers to address concerns related to mental health and family dynamics. - Advocate for residents’ rights and contribute to improved social conditions. - Participate in regular peer-to-peer support meetings with fellow professionals. - Engage in continuous career development opportunities to enhance skills. Requirements - Bachelor's or Master's degree in Social Work from an accredited institution. - Registered member of the Ontario College of Social Workers and Social Service Workers. - Experience in case management is essential; experience with elderly populations is an asset. - Strong communication skills, both verbal and written, for effective interaction with residents, families, and staff. - Ability to work independently as well as collaboratively within a team environment. - Demonstrated commitment to ongoing learning - Current Vulnerable Sector Check and clear 2 Step TB tests prior to start date - Professional and Commercial Liability Insurance - Experience in long-term care/ psychiatric / medical social work practice - Certificate in Gerontology or Dementia Care - MoCA Montreal – Cognitive Assessment trained and certified - Gentle Persuasive Approach - PIECES trained - Palliative Care If you are passionate about making a difference in the lives of vulnerable elderly individuals, we invite you to apply today and join our dedicated team at Age-Wise Solutions Inc.
