Job Summary We are seeking an experienced Registered Nurse (RN) or Registered Practical Nurse (RPN) to complete in-home Activities of Daily Living (ADL) assessments for clients requiring long-term care support, insurance evaluations, and care planning. This is a flexible, independent contractor role ideal for nurses with experience in community care, case management, or long-term care who are comfortable working autonomously and producing high-quality clinical documentation. This role is best suited for nurses with prior experience completing independent assessments and narrative reports for third-party stakeholders (insurance, legal, or care coordination). Responsibilities Conduct in-person ADL assessments in client homes across the Greater Toronto Area (GTA). Evaluate functional status including: Mobility and transfers, Personal hygiene and grooming, Toileting and continence, Feeding and meal preparation, Cognitive status and safety awareness. Complete standardized assessment tools (e.g., Barthel Index, Katz ADL, or equivalent). Develop clear written reports outlining: Functional limitations, Level of assistance required, Risk factors and safety concerns, Recommendations for care and support. Communicate findings with case managers, care coordinators, or third-party stakeholders. Maintain accurate and timely documentation in accordance with qualified standards. Travel to clients’ homes (mileage compensated where applicable). Qualifications Current registration in valuable standing with the College of Nurses of Ontario (CNO) RN or RPN designation Minimum 3–5 years of clinical experience, preferably in one or more of: Community nursing, Home care, Long-term care, Rehabilitation or geriatrics. Strong understanding of ADL/IADL assessment frameworks Excellent clinical judgment and observational skills Experience completing formal assessment reports (insurance, legal, or LTC eligibility an asset) Valid driver’s license and access to a reliable vehicle Professional liability insurance (or willingness to obtain) Pay: $75,000.00-$80,000.00 per year Benefits: Flexible schedule Mileage reimbursement In office parking Work Location: Hybrid remote in Toronto, ON M2N 6K8 Apply on Kit Job: kitjob.ca/job/2pn9pd
