Part-Time Nurse Practitioner Youth Wellness (Whitby)

Enhance youth wellness with Durham Community Health Centre as a part time Nurse Practitioner. Provide trauma-informed care and support across multiple locations, fostering youth engagement in health decisions.

As a Nurse Practitioner (NP) within the Youth Wellness Hub, youll deliver comprehensive primary care addressing mental health, substance use, and overall wellness. Your role includes direct collaboration with multi-disciplinary teams and external partners for integrated care. Youll play a vital part in shaping equitable health outcomes for diverse youth populations.

Key Responsibilities: • Deliver youth-centered primary care and wellness services • Prescribe and manage pharmacotherapy within NP scope • Conduct intake assessments and support care navigation • Facilitate virtual and community-based care options • Advocate for youth involvement in their health decisions

Requirements: • Master’s Degree in Nursing with NP certification • Minimum 3 years of primary care experience • Current CNO registration as a Nurse Practitioner • Experience in community health settings preferred • Full COVID-19 vaccination required
