Part-Time Nurse Practitioner Youth Wellness Hub (Whitby)

Become a vital part of the Durham Community Health Centre as a part-time Nurse Practitioner at the Youth Wellness Hub. Deliver compassionate, youth-centered care and wellness support with a focus on mental health, substance use, and physical health needs. As the Nurse Practitioner (NP), you will join an inter-skilled team in Oshawa South, working directly with youth facing various health challenges. Your responsibilities will encompass providing clinical care, conducting assessments, and facilitating community-based services. Expect to work a flexible schedule, including evenings and weekends, with an emphasis on trauma-responsive and culturally appropriate care. Key Responsibilities: • Deliver youth-centered primary care and wellness services • Prescribe medications within NP scope of practice • Conduct intake assessments and support warm handoffs • Collaborate on integrated care plans with team members • Create a safe environment for youth engagement Requirements: • Master’s Degree in Nursing with NP certification • Minimum 3 years experience in primary care • Registered as an NP in Ontario • Experience with youth and marginalized populations preferred • Excellent communication and collaboration skills Empower the youth of Durham through comprehensive health solutions and advocacy in your role at DCHC.
