Nurse Practitioner for Youth Wellness Services (Whitby)

Join the Durham Community Health Centre as a Nurse Practitioner dedicated to youth wellness. This part-time role involves providing integrated health services addressing mental health, sexual health, nutrition, and more.

As an NP at the Youth Wellness Hub in Oshawa, you will work 17.5 hours weekly, including evenings and weekends. Your role is critical in delivering trauma-informed primary care, conducting assessments, and engaging youth in their wellness journeys. Collaborate with a committed inter-skilled team to ensure comprehensive care while supporting clients from diverse backgrounds.

Key Responsibilities: • Provide youth-centered primary care and wellness education • Manage pharmacotherapy and mental health medications • Facilitate virtual and community-based care • Support care coordination and navigation for youth • Uphold principles of equity and inclusivity

Requirements: • Master’s Degree in Nursing with NP qualification • Three years in primary care for complex health needs • Current Ontario NP registration • Knowledge of Durham Regions health resources preferred • Strong interpersonal and team collaboration skills
