Registered Dietitian for Diabetes and Primary Care (Toronto)

As a Clinical Dietitian, you will work with patients at both the Sunnybrook Diabetes Education Centre and the Sunnybrook Academic Family Health Team. The role includes evaluating nutrition habits, implementing medical nutrition therapy, and facilitating community outreach education. This position emphasizes teamwork and the application of advanced nutritional strategies to support patient health.

Key Responsibilities: • Evaluate patient dietary habits and develop tailored plans • Offer medical nutrition therapy and lifestyle management • Educate in individual and group settings on diabetes • Document all assessments in EMR (PSS) • Develop community health promotion programs

Requirements: • Bachelor’s degree in Human Nutrition or equivalent • Registration with the College of Dietitians of Ontario • 2+ years of diabetes care experience • Proven ability to work independently • Excellent time management and communication skills
