As a full-time Case Manager, you will support clients in mental health by conducting assessments in various settings including encampments, hospitals, and shelters. Collaborating within the Assertive Housing First Team, your role encompasses developing care plans, managing medications, and providing community-based support. Relationships with clients will be built on trust, necessary for their recovery and stabilization. Key Responsibilities: - Conduct face-to-face assessments at client locations - Develop and manage coordinated care plans - Administer medications with team psychiatrist - Provide comprehensive community-based case management - Link clients to essential community services Requirements: - Bachelor’s Degree in Nursing - Registered with the College of Nurses of Ontario - Minimum 2 years’ nursing experience - Preferably skilled in psychiatric nursing - Strong knowledge of the Housing First model Make a meaningful impact in the lives of individuals facing mental health challenges and support their journey to recovery.
